                        <content:encoded><![CDATA[https://www.usatoday.com/story/news/health/2023/01/09/mariujuana-surgery-anesthesia-guidelines/10988361002/<br><br><b>Before undergoing surgery, patients should tell their anesthesiologist if they&#039;re a heavy cannabis user and, according to new guidelines, anesthesiologists should be asking about use. <br><br>Cannabis use, particularly when it&#039;s regular and heavy, can leave patients in more pain than normal after surgery, and getting high just before an operation can put a patient at risk for heart problems, even a heart attack, said Dr. Samer Narouze, president of the American Society of Regional Anesthesia and Pain Medicine.<br><br>The society released the guidelines Jan. 3, and they are intended to be both educational and instructive, Narouze said. While medical schools are now starting to teach about cannabis, most practicing doctors learned little about the drug or the body&#039;s reaction to it during their training.<br><br>&quot;It&#039;s a problem for every practicing anesthesiologist. They have this patient and they just don&#039;t know what to do,&quot; said Narouze, also chairman of the Center for Pain Medicine at Western Reserve Hospital in Cuyahoga Falls, Ohio. <br><br>The guidelines recommend anesthesiologists ask patients whether they use cannabis, how much, how often, how recently, what type of marijuana and whether it was smoked or ingested. The guidelines also call for postponing elective surgery if a patient arrives high and educating pregnant patients about the risks of using marijuana on their baby. <br><br>Some heavy marijuana users may require more anesthesia to fall asleep or keep them asleep, he said. Heavy use may leave patients more agitated during recovery, and someone acutely intoxicated with cannabis may feel confused, have a racing heart and panic episodes, and cannot appropriately consent to surgery, Narouze said.<br><br>A simple conversation before the operation can help the anesthesiologist provide appropriate pain control and help the patient understand their risks, Narouze said. A heavy user might need more monitoring during recovery and their discharge could be delayed.<br><br>&quot;If the anesthesiologist is aware of the risks,&quot; he said, &quot;they can put the pieces together and manage the patient well.&quot;<br><br>Marijuana use can leave patients in more pain than normal after surgery <br>Anesthesiologists always consider &quot;multi-modal&quot; approaches to pain management to minimize side effects, said Dr. Peter Yeh, an anesthesiologist at the University of California San Francisco. But in a regular, heavy marijuana user, he pays extra attention to other classes of medications, including nerve blocks or an epidural.<br><br>A small dose of the THC in cannabis reduces pain, but in high doses, it can increase it, Narouze said. The same is true for nausea. Emergency rooms have seen a growing number of patients with hyperemesis – essentially, nonstop vomiting – caused by excessive cannabis use.<br><br>Narouze said edibles increase the risk for hyperemesis because they take longer to lead to a high, so people think they haven&#039;t yet eaten enough. Smoking causes a quick high that also fades rapidly, while edibles take longer to have their longer-lasting effect. <br><br>A heavy cannabis user might also miss one or two regular doses during a long surgery and recovery, leading to more pain, Narouze said. <br><br>Medical cannabis use is legal in 37 states and recreational use in 19, plus the District of Columbia, according to the National Conference of State Legislatures.<br><br>Yeh said his patients receive counseling about cannabis use before surgery, so he&#039;s never had a surgical patient who&#039;s actively high. But he takes a patient&#039;s marijuana use into consideration when he plans their surgery and recovery.<br><br>&quot;It&#039;s definitely dose dependent,&quot; he said. &quot;It&#039;s just something we have to take into consideration when we take care of you.&quot;</b>]]></content:encoded>

                        <content:encoded><![CDATA[<span style="color:turquoise"><span style="font-size:12ptpx"><b>Palm kernel oil is the most common oil in microwave popcorn.</b></span></span><br><br>Scientists have shown how a fatty acid found in palm oil can encourage the spread of cancer, in work that could pave the way for new treatments.<br><br>The study, on mice, found that palmitic acid promoted metastasis in mouth and skin cancers. In future, this process could be targeted with drugs or carefully designed eating plans, but the team behind the work cautioned against patients putting themselves on diets in the absence of clinical trials.<br><br>“There is something very special about palmitic acid that makes it an extremely potent promoter of metastasis,” said Prof Salvador Aznar-Benitah, of the Institute for Research in Biomedicine (IRB), Barcelona. “I think it is too early to determine which type of diet could be consumed by patients with metastatic cancer that would slow down the metastatic process.”<br><br>The study adds to emerging evidence that diet can be used to enhance existing cancer treatments because certain nutrients are disproportionately relied on by tumour cells, or are required at critical stages such as metastasis.<br><br>The study built on previous work by the same team showing that, within a tumour, just a small subset of cells have the capacity to spread by travelling out of the tumour, reaching other organs and colonising them. These specialised cancer cells appeared to rely particularly heavily on fatty acids and the latest work narrowed this down to palmitic acid, which is found in palm oil – but also in a wide variety of foods such as butter and olive oil.<br><br>The study, published in Nature, found that when palmitic acid was supplemented into the diet of mice, mouth and skin cancers were more likely to spread. Other fatty acids called oleic acid and linoleic acid – omega-9 and omega-6 fats found in foods such as olive oil and flaxseeds – did not show the same effect. Neither of the fatty acids tested increased the risk of developing cancer in the first place.<br><br>The study suggested that exposure to palmitic acid caused changes to the function of genes in cancer cells that allowed them to sense fatty acids and consume them more efficiently. The presence of palmitic acid also appeared to send cancer cells into a “regenerative state” allowing them to form signalling networks beyond the tumour, which is known to be a crucial step towards spreading.<br><br>Metastasis of cancer remains the main cause of death in cancer patients and the vast majority of people with metastatic cancer can only be treated, but not cured.<br><br>By understanding what cancer cells need to make this leap, the scientists also identified ways to block the process and are planning a clinical trial of proteins that interfere with the tumour response to palmitic acid.<br><br>Prof Greg Hannon, director of the Cancer Research UK Cambridge Institute, said: “This is a rigorous and comprehensive study that suggests that exposure to a major constituent of palm oil durably changes the behaviour of cancer cells, making them more prone to progress from local to potentially lethal metastatic disease. Given the prevalence of palm oil as an ingredient in processed foods, this study provides strong motivation for further study on how dietary choices influence the risk of tumour progression.”<br><br>Helen Rippon, chief executive at Worldwide Cancer Research, said: “This discovery is a huge breakthrough in our understanding of how diet and cancer are linked and, perhaps more importantly, how we can use this knowledge to start new cures for cancer. Metastasis is estimated to be responsible for 90% of all cancer deaths – that’s around 9 million deaths a year globally. The samples, she said, “provided evidence of vitamin E acetate at the primary site of injury in the lungs.”<br></span><br>Samples taken from the patients were also tested for plant oils, petroleum distillates like mineral oil and other potentially harmful substances, which were “notably not detected,” the C.D.C. said. The findings are being published in Friday’s Morbidity and Mortality Weekly Report.<br><br>About 70 percent of the patients are male, 79 percent are younger than 35 and 86 percent say that they have vaped THC.<br><br>Many of the products used by those who became ill were illicitly obtained, public health experts have said, by patients who bought them from friends or on the street. Vaping oils typically include other additives, solvents and flavor enhancers. Vitamin E acetate is sometimes added to dilute the THC to increase profits or as a thickening agent. Health investigators have said since nearly the beginning of the outbreak in mid-August that some ingredients, including vitamin E acetate, could be responsible for some of the lung illness cases.<br><br>State health officials in New York had first identified vitamin E acetate from several samples collected in August that were analyzed by the Wadsworth Center lab.]]></content:encoded>

This exposes the goodies to your upper sinuses which I know better than anyone is connected to your brain. I find I get 50% more benefit from a hit of weed smoking it this way! Had stomach cramps sat pm and Sunday, was really hoping they were totally from kratom letting go<br><br>Turns out yes they were. So those 2 grammers were enough to stop RLS and let me get some rest<br><br>And also small enough that my body continued it's cleanse.<br><br>So today is #5, I think im done.
